In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
- Photo ID
- Insurance card
- New Patients: download a patient registration forms if you cannot complete them on the Prime Patient Portal
Your visit will start and end with the Appointment Desk. We will engage you from the very start with a warm greeting and a smiling face. Women's Care appointment secretaries are skilled in finding the appointment type and time that works for you. If you are coming in for your annual exam, please prepare for this exam by knowing your family history of breast and other cancers. IMPORTANT: If you are a New Patient or you haven't been here for more than 3 years, please download our Patient History Form if you cannot complete the forms on the portal, complete it in full and mail it our clinic one week prior to your appointment. Also, please have any medical records sent to us one week prior to your appointment (Release Records TO Women's Care from another doctor). If either of these items are not here on the day of your appointment, you may be asked to reschedule.
Before you see the physician or nurse practitioner, you will see a kind and caring nurse who will call you by name, ask initial questions, enter your information in the computer, and take your vitals.
Nurse Practitioner or Physician
Each Women's Care nurse practitioner and physician is eager to address your concerns and provide you with excellent comprehensive care.
For women age 40 years and older, digital mammography may be a required part of your annual screening. We offer digital mammography onsite, and our three technicians have over 30 years of experience.
Our ultrasound department has over 30 years of experience. Routine diagnostic as well as Gender Determination and 4D scans are available.
For women age 65 years and older or for those with risk factors, bone densitometry may be an important measure to consider for your health. We offer the convenience of having your bone scan onsite.
Increasingly, insurance concerns are an important part of healthcare. Our insurance department is available to help you with questions or concerns
The onsite laboratory saves you precious time by providing an opportunity to draw labs right after the physician orders them at your appointment. Our lab consistently receives 100% COLA certification.
Even before or after your visit, we care about your health concerns. Telephone Triage Nurses are available to return your calls during office hours. They are trained to handle your medical concerns and to help determine the urgency of your problems.
